High-Risk Exposures That Qualify for PEP

The following exposures are generally considered high-risk and qualify a person for PEP:

Sexual Exposures

  • Unprotected receptive anal intercourse (highest risk category)
  • Unprotected insertive anal intercourse with an HIV-positive partner
  • Unprotected vaginal intercourse with a confirmed HIV-positive partner
  • Unprotected sex with a partner whose HIV status is unknown (especially in high-prevalence settings)
  • Condom breakage or slippage during sex with a person whose status is positive or unknown
  • Sexual assault (this always qualifies, regardless of the perpetrator's known status)

Occupational Exposures

  • Accidental needlestick or sharps injury involving a patient's blood
  • Splash of a patient's blood or body fluid onto an open wound, cut, or mucous membrane
  • Exposure during blood draws, surgery, or laboratory work in healthcare settings

Other Exposures

  • Sharing needles or drug preparation equipment with an HIV-positive person
  • Transfusion of blood from an unscreened or unverified source (rare in India's modern healthcare system)

Lower-Risk Exposures That May Not Require PEP

Not every exposure justifies a full 28-day PEP course. A qualified doctor will assess the following as generally lower risk:

  • Oral sex (giving or receiving) — risk is very low but not zero
  • Needlestick from a needle that has not been used by an HIV-positive person
  • Exposure to urine, saliva, or tears (which do not transmit HIV)
  • Casual contact: handshaking, hugging, sharing food or utensils
