When it comes to HIV, most people think in terms of "safe" and "unsafe" without understanding the spectrum of risk between those two poles. The reality is that HIV transmission probability varies enormously based on the type of sexual act, the role you played, and several biological factors. Understanding actual data helps you make rational decisions — including whether to seek PEP — rather than reacting to fear alone.

The risks below are expressed as transmissions per 10,000 exposures with an HIV-positive partner who is not on treatment (i.e., has a detectable viral load). These are per-act estimates from scientific literature.

Highest Risk: Receptive Anal Intercourse

Receptive anal intercourse (the "bottom" role) carries the highest risk of HIV transmission of any sexual act. Estimated risk: 138 per 10,000 exposures (approximately 1.4%) per act with a positive partner. Lower Risk: Insertive Vaginal Intercourse

For a male partner during vaginal intercourse with an HIV-positive female: 4 per 10,000 exposures (0.04%). Circumcision reduces this risk by approximately 60%.

Very Low Risk: Oral Sex

Oral sex carries a very low but non-zero risk of HIV transmission. Receiving oral sex (fellatio) from an HIV-positive person: risk is estimated at less than 1 per 10,000 exposures. Performing oral sex on a male HIV-positive partner: similarly very low. Risk increases with open sores, bleeding gums, or ejaculation in the mouth. • High viral load in the positive partner: Risk is up to 8x higher when the source partner has a high viral load (untreated HIV)
• Co-existing STIs: Having gonorrhoea, syphilis, herpes, or chlamydia creates open sores and inflammation that dramatically increases transmission probability
• Bleeding or trauma: Any abrasion, bleeding, or roughness during sex elevates risk
• Ejaculation: Internal ejaculation significantly increases receptive partner risk

Factors That Reduce Risk

• Undetectable viral load (U=U): A positive partner who is on ART with an undetectable viral load cannot sexually transmit HIV. This is one of the most important advances in HIV science.
• Condom use: Reduces transmission risk by approximately 85% when used correctly and consistently
• Circumcision: Reduces insertive HIV risk by approximately 60%

Does a Low Percentage Mean You Are Safe?

This is a crucial question. A "4 in 10,000" risk sounds small — and it is, per act. But most people do not have unprotected sex with an HIV-positive partner only once. Repeated exposures multiply the cumulative risk substantially. Additionally, "low probability" is not the same as "zero probability."
