What Happens If You Miss a PEP Dose?
Taking medication perfectly on time every single day for 28 days is difficult. It is incredibly common for patients to forget a dose, fall asleep early, or get stuck without their pills. When a patient realizes they have missed a PEP dose, panic usually sets in immediately.
If you have missed a dose, the most important thing is to stay calm and take the right corrective action. Here is the strict medical protocol on what to do.
Why Timing Matters So Much
PEP drugs work by keeping a constant, high level of antiretroviral medicine in your bloodstream. This chemical barrier prevents the HIV virus from copying itself and establishing a permanent infection. When you miss a dose, the level of medication in your blood drops. If it drops too low, the virus has a window of opportunity to take hold.
The "12-Hour Rule"
The action you should take depends entirely on how late you are. The standard medical guideline is based on the 12-hour mark for once-daily pills.
- If you are less than 12 hours late: Take the missed dose immediately as soon as you remember. Then, take your next dose at the usual, regularly scheduled time. (Yes, this means the two doses will be closer together than usual, and that is okay).
- If you are more than 12 hours late: Do not take the missed dose. Skip it entirely. Wait and take your next dose at the regularly scheduled time. Never double up or take two pills at once to make up for a missed dose. A double dose will not give you extra protection, but it will significantly increase toxic side effects and liver strain.
- Note: If you are taking a twice-daily regimen, the window is usually 6 hours instead of 12.
Will One Missed Dose Cause HIV?
Missing a single dose is not ideal, but it does not mean the treatment has failed. Modern PEP regimens (like Dolutegravir-based treatments) are robust and have some "forgiveness." However, every missed dose chips away at the overall efficacy (which is normally 80-99%).

- Keep emergency pills on you: Keep a spare pill in a secure container in your bag or car in case you are unexpectedly delayed away from home.
- Tie it to a habit: Take it right after brushing your teeth or right after dinner.
