Does HIV Risk Differ Between Insertive and Receptive Partners?

The Biology of Transmission

A common question among patients is whether the risk of contracting HIV is the same for both partners during intercourse. The short answer is no. While both partners are at risk, the biological mechanics of transmission make the risk significantly different depending on whether you are the insertive (top) or receptive (bottom) partner.

Risk for the Receptive Partner

The receptive partner during anal intercourse faces the highest risk of HIV transmission of any sexual act. The mucosal lining of the rectum is very thin and contains a high concentration of immune cells—the very cells HIV targets. Small, often unnoticed micro-tears can occur during intercourse, providing direct access to the bloodstream for the virus present in seminal fluid.

For vaginal receptive intercourse, the risk is lower than anal receptive, but still significant. The vaginal lining provides a better barrier, but factors like inflammation, untreated STIs, or menstruation can increase the risk.

Risk for the Insertive Partner

The insertive partner is at a lower risk compared to the receptive partner, but the risk is absolutely not zero. HIV can enter the insertive partner's body through the opening of the urethra at the tip of the penis, through the foreskin (if uncircumcised), or through small cuts, scratches, or sores anywhere on the penis.

How to Mitigate the Risks

Regardless of your role, protection is vital:

• Barrier Methods: Condoms provide a physical barrier that drastically reduces transmission risk.
• Lubrication: Water-based or silicone-based lubricants reduce friction, decreasing the likelihood of micro-tears.
• PrEP: Pre-exposure prophylaxis protects regardless of sexual role.
