The HIV virus does not survive well outside the body, making environmental transmission extremely difficult.

However, a theoretical risk exists if equipment is shared and not properly sterilized between clients. This is mostly a concern in unlicensed settings, prisons, or amateur "stick-and-poke" scenarios.
